WASHINGTON, NJ [August 29, 2005] – The Warren County Health Department needs the help of residents of Warren County to complete a Behavioral Risk Survey. On August 29th a Behavioral Risk Survey was mailed to 2,500 randomly selected county residents. This survey included questions on diet, smoking habits, physical exercise, health screenings and safety issues. The surveys are anonymous and to be completed by residents over the age of 18.
If you receive one of the Behavioral Risk surveys please complete the survey and return it in the self addressed stamped envelope. Your honest and truthful answers to the questions on the survey are very important to our planning efforts. As a resident of Warren County, we need your help to develop a useful health plan that addresses the health issues we face in our local communities. |

The more surveys returned the better information we will have that is representative of Warren County residents. Surveys are to be returned by September 15, 2005.
Any Warren County resident who did not receive a survey but wishes to participate can complete a survey online. The survey is available at www.co.warren.nj.us/health.
